I think it depends how recently the death was. The age at death should be in the index along with vol and page numbers from about 1870 ish I think. |

That has always been there Deanna, as people have said you only need to complete that if you have not been able to complete the page/vol ref numbers or if it is within the last 50 years. Have another look at the form. |

Deanna, it is easily done. Have another look at the form, my guess is that you missed the little box you need to tick which asks if you have the ref numbers for it. I have done it myself. |
