Multiple marriage?

Hi i was looking at a birth record, and something has confused me so i thought i would ask for several pieces of help.

Firstly, if on the birth certificate the surname at marriage is different to maiden name and full name, does that mean they have been married more than once?

Secondly, If teh parent and child have the same surname abnd the mothers maiden name and name at marriage are different is this just put down to teh fathers surname.

sorry wasnt very clear there, on teh birth certificate there is no name for teh father, which is why i was question the three names for the mother being shown.
