Good luck Florence for your op. It was the number of ethnic choices got me, when I read therm out OH thought I was making them up! I would have expected, european, carribean, african, asian, chinese but 16!!!!!

hi chris, i am going in onthe 29nov for my sinus operation and they sent me loads of paperwork and like you had a huge form to complete. the questions were about background, religion, race etc and i think it is quite standard to get this up here in scotland. i never put any religion down as when you do, suddenly when you least expect it, some chaplin appears before your op and prays for you. i dont so much have a problem with that except they never ask if you mind, they just suddenly start their sermon, something i object to. so i leave it or put other and they never ask me what strangley enough!

i will of course be not on here for a week probably, but will look forward to catching up on anymore changes when i return.lol

the NHS round here are going all computerized
so anybody with the password including you and your GP
have access to all your medical notes

and it will save hours of paper work

I have had quite a few hospital appointments myself this year and have been sent this form for every one of them. Like you I think it should be on my file anyway, but I always fill it in (missing out any questions I don't want to answer) take it with me, but am never asked for it! I think they send them out just to cover themselves in case there isn't one in your file.

had to fill in a form giving all my details (think after 6 months in there they would know them all by heart now) for an appointment tomorrow. However when I came to Ethnic background I was given 16 possibilities and one - 'refuse to answer'. Anyone else come across a form like this before?
