Can you post the full info so we can take a look

It could be the later info for GRO records that you need to buy a copy of a cert
Would need to see it in context to advise

Found this by googling and it is part of the GRO record needed to buy a copy of the cert from the GRO

Follow the order form with requesting a death cert and fill in the boxes with the relavant info from the record above . Don't forget to tick the box that asks if the GRO ref is known
As the death is 2003 the form will adjust so you can put in the district nos etc . It won't need all the refs quoted in the GRO record
