Thank you everyone for your replies. I have traced Frances Moffat with her son Harold and daughter in law Phyllis, and have the death records for each of them.
I don’t think I will ever find out what happened to Olwyn, her name would have been changed by her adoptive family

I can't find a Frances born on that date registered after 1984, so should narrow the search a bit.

I am presuming that whoever registered it did indeed know her DOB.

all deaths have to be registered and green slip issued so the undertaker can perform the funeral arrangements.
She must have been registered but its under what name !!
