Who registered Hannah's death?

How was she described? i.e. 'wife' or 'widow'

Could his death certificate not be in his full name e.g. could he be registered as Thomas Smith rather than Mark Thomas Smith. There is a tree on Ancestry which doesn't have much information about him but has his name (if it is the same man) as Mark Thomas Henry Smith. There is a death of a Thomas H. Smith registered in Aston in first quarter of 1920 with an age of 68, which would give a birth year of 1852. Just thought it might be another possibility.

Of course Smith is a terrible name to research - my grandmother was a Smith so I know what a nightmare it can be to find them.
